Age Concern – Hospital Discharge Service

Summary of aims & objectives:
- To provide short-term support for disadvantaged/frail elderly persons just after discharge from hospital
- To prevent the necessity for re-admission due to socio-economic circumstances
- To explore the clients’ perception of need
- To facilitate inter-organisational co-ordination and co-operation
- To help improve the individual’s quality of life
Services: We provide short-term intensive support for anyone 60 years old and over on discharge from hospital. The service is available 365 days a year apart from bank holidays. It is predominately a 9am to 5pm service seven days a week offering practical and emotional support and also liaises between the client and statutory, private or voluntary organisations.
The service does not provide specialised support to EMI clients. However, it does support clients with a mental health illness who have accessed acute services.
Referrals:
As long as the criteria is met anyone can access the service [with the client’s consent] via the telephone or in person.
